I am at least 18 years old and I own or have permission from the owner of my registered dog to participate in the Boo-le-Bark on the Boulevard Dog Parade. *

I can prove that my dog is current on his or her rabies vaccination. *

I acknowledge that I am responsible for my dog’s health and well-being while participating in the Dog Parade. *

I hereby agree that I am responsible for the actions of my dog and myself during the parade and further agree to release, indemnify, and hold harmless the Boo-le-Bark on the Boulevard Dog Parade, its sponsors and organizers, Athenspets, Inc., and Athens-Clarke County, Athens, Georgia from any liability arising out of the participation, actions, or omissions of my dog or myself. *

I give permission to use photos taken during the Boo-le-Bark on the Boulevard Dog Parade for promotional purposes. *

I agree to have my dog leashed and under control at all times. I acknowledge that I am responsible for picking up all of my dog’s waste. *
